About the opportunity:

We are searching for a candidate to fulfill FT Team Clerk position with our South Pasco Field Team.  The shift is 8:30 am to 5:00 pm, Monday to Friday.  The Team Clerk provides clerical support of an assigned Team and may process incoming telephone calls, greet visitors, and perform other assigned clerical functions on an as needed basis and scheduled in advance.

Education / Skill Requirements:

•The primary function of the Team Clerk is to provide support for the Nursing and Social Work Supervisor in any capacity required, which includes responsibilities as listed below.

•Assists in computer input and update of patient record information.

•Runs daily admission statistic “patient list” report and hospice daily short census report and gives to Social Work Supervisor

•Fax face sheets and updates for facility’s pharmacy and Pharmacia

•Verifies Admissions and creates Patient Chart, makes labels for chart

•Completes requests from staff for printing of face sheets(2)placing one in the “Face Sheet Book” and a medication profile (2) on for the Case Manager and one for the chart

•Maintains Face Sheet Book to keep patient list up to date

•Print face sheet and med profile for Pool nurses that are not using laptop

•Receives email from Nursing Supervisor when admission/recertification has been completed so that PPOT can be created and printed

•Make copies of all Certifications and Re-certifications for Case Manager to take to ECF and ALFs

•Initiates the PPOT and notifies Case Manager for any open areas prior to printing.  Print and file PPOT in patient chart once signed and received.

•Verifies and processes IDG for completeness following TEAM.  Returns incomplete IDG’s to Supervisor for any incomplete information to be completed by the Case Manager and/or Social Worker

•Prepares TEAM sheet – Send original with attendance sheet to Medical Records

•Maintain log of H&P requests until received.  Once received, places with filing for patient chart

•Oversee and maintain all medical record charts

•Oversee, coordinate and communicate with Volunteer staff regarding documentation to be filed in patient and bereaved client charts ( JSA Monthly Summary, Modified Orders)

•Close out all inactive charts to include bereavement and CAP.  Checks all documents and follow up and reconcile modified orders

•Prepare all files and documents for internal audit working with Data Entry Clerk against itineraries
